April ICD-10-CM Update Comes in Like a Lamb

Small but significant changes to the code set are on the way.

ICD-10-CM has released its April 1 update to the code set for fiscal year (FY) 2025. There are no code changes in the update, but there are several verbiage changes to the Index and Tabular List that may change coding. Medical coders and auditors should be aware of these changes to safeguard the integrity of patient data and claims payment.

Changes to the Tabular List of Diseases and Injuries

Chapter 1

Under Tuberculosis (A15-A19), ICD-10-CM adds a Use Additional code note for associated cachexia, coded to E88.A (Wasting disease (syndrome) due to underlying condition).

Under B59 (Pneumocystosis), ICD-10-CM revises an inclusion term to correct the spelling of jiroveci to jirovecii.

Chapter 4

Under E23.0 (Hypopituitarism), you’ll find a new Use Additional code note for cachexia (E88.A).

Chapter 10

Under J17 (Pneumonia in diseases classified elsewhere), there is also a correction of the spelling for jirovecii in the Excludes1 note.

Under J65 (Pneumonoconiosis associated with tuberculosis), you’ll find a new Use Additional code for associated cachexia (E88.A).

Chapter 11

Under K56.41 (Fecal impaction), ICD-10-CM has deleted the Excludes1 note for constipation (K59.0-).

Under K72.- (Hepatic failure, not elsewhere classified) and K73.- (Chronic hepatitis, not elsewhere classified), you’ll find a new Use Additional code note for ascites (R18.8) is added.

Under K90.82- (Short bowel syndrome), you’ll also find a new Excludes1 note for postsurgical malabsorption, not elsewhere classified (K91.2).

Under K90.822 (Short bowel syndrome without colon in continuity), ICD-10-CM has also added a Use Additional code note for colostomy status (Z93.3).

Chapter 14

For N18.- (Chronic kidney disease (CKD)), ICD-10-CM has revised the Use Additional code note to include associated cachexia (E88.A).

Chapter 15

Under O04.- (Complications following (induced) termination of pregnancy), ICD-10-CM has revised the wording for the Excludes2, removing the word “uncomplicated.” The note now reads: “Excludes2: encounter for elective termination of pregnancy, uncomplicated (Z33.2).”

Under O26.64- (Intrahepatic cholestasis of pregnancy), you’ll find a new Use Additional code note for hepatic obstruction (K76.89).

Chapter 19

Under Injury, poisoning and certain other consequences of external causes (S00-T88), you’ll see the Excludes1 note for birth trauma (P10-P15) and obstetric trauma (O70-O71) is now an Excludes2 note.

Under S06 (Intracranial injury), ICD-10-CM has revised the note that currently reads “The appropriate 7th character is to be added to each code from category S06” to “The appropriate 7th character is to be added to each code from category S06, except as noted below.” This refers to the 7th character note that follows, which tells you “7th characters D and S do not apply to codes in category S06 with 6th character 7 - death due to brain injury prior to regaining consciousness, or 8 - death due to other cause prior to regaining consciousness.”

Chapter 21

Under Z3A (Weeks of gestation), ICD-10-CM has revised the Code first note for obstetric condition or encounter for delivery to include codes O94-O9A.

Changes to the Index to Diseases and Injuries

Accordingly, there are revisions, additions, and deletions under:

D: Decrease and Dissection

I: Infection, infected, infective (opportunistic)

M: Myocarditis

P: Pneumocystis and Pneumonia
